Does anyone know if the MSL is specific to the device or is it account wide? And also is it reset by flashing a custom ROM, or hard programmed into the phone?

The MSL or Master Subsidy Lock code is specific to your device and is persistent through all operations. It won't change unless it is changed by the user.
